Steps to Participate in ASI Sports
Step 1: Get a Physical
A current sports physical is required for all CUSD secondary student-athletes.
- The physical is valid for one year from the exam date and covers participation in all sports.
- If your child’s physical will expire before the season ends, a new physical must be completed before the season begins to ensure eligibility.
Step 2: Review Safety Information
CUSD wants to make sure families understand both the benefits and the risks of participating in school sports. We’ve gathered important safety information to help you stay informed and to give you practical ways to support your child’s health and well-being.

Step 3: Online Registration

Go to HomeCampus and create a parent account.

Use your email address to register.

Follow the prompts to connect your account to your student.

Acknowledge the Safety Information.

During registration, you’ll be asked to review the sports safety materials provided by CUSD.

Check the box to confirm that you and your student have read and understood this information.

Upload Your Child’s Current Physical.

Scan or take a clear photo of the completed physical form.

Upload the file directly into your child’s Home Campus account.

Make sure the physical will be valid through the entire season.

✅ Once this step is complete, you should receive an email notification.
Your Athletic Director will review your registration and determine if your child is cleared to move forward in the sports participation process.
